FAR works with a trusted insurance broker to connect families to the policy that is right for their situation.

Applications are accepted all year.

 

New families will be added during the months of November & December for coverage starting the following year.

In keeping with our mission and core values, the Foundation for Autism Resources is committed to working to ensure ABA therapy services for qualifying children ages 2- 12.

The following are requirements that must be met to receive FAR’s services:

  • Child must qualify for and currently be enrolled in Indiana Children’s Special Health Care Services.

  • Child must have a diagnosis of autism, Aspergers, PDD-NOS, childhood disintegrative disorder or Rett’s syndrome and be between the ages of 2 and 12.

  • Commitment to receive 30 - 40 hours per week of ABA therapy following recommended weekly hours by a BCBA as appropriate for child’s maximum progress.

To be considered for financial assistance, you must supply the following:

  • Completed and signed application form

  • Previous year’s tax return (Federal and State)

  • Copies of earnings statements for each person in the household for the past 3 months (pay stubs, Social Security statements, unemployment, retirement, pensions, etc.).

  • One copy of each of your last three bank and investment account statements.

  • Personal letter explaining your need for financial assistance.

  • One copy of any diagnostic/evaluation reports regarding your child’s diagnosis

  • Letter of support from the assisting party if the household is receiving assistance from family or friends.
